NR509 Week 4 Discussion: Client Challenges

Course
NR509 Advanced Physical Assessment

Preparing the Discussion

Follow these guidelines when completing each component of the discussion. Contact your course faculty if you have questions.

General Instructions

Some clients present specific challenges during the interview and physical examination. Nurse practitioners (NPs) must be able to identify behaviors that impede the assessment and consider strategies to communicate effectively with these clients. This week, you will create a scenario that could occur in NP practice in which a client displays a challenging behavior as assigned below.

Your topic is assigned based on the first letter of your first name. For example, if your first name is Grant then your assigned topic is communicating with a flirtatious client.

The First Letter of Your First NameTopic
A – DArgumentative client
E – HFlirtatious client
I – LTalkative client
M – PClient who displays racist or discriminatory behavior
Q – TAdolescent client who is reluctant to answer questions
U – ZViolent client

Include the following sections (detailed criteria listed below and in the grading rubric):

  1. Application of Course Knowledge: Answer all questions/criteria with explanations and detail.
    1. Create a Realistic Scenario

      • Define the setting (clinic, telehealth, hospital, etc.) and type of encounter (routine check-up, follow-up, acute visit).

      • Tip: Include details about the client and the NP to make your scenario realistic and engaging.

    2. Describe the Client’s Challenging Behavior

      • Explain how the client exhibits the behavior assigned (e.g., argumentative, flirtatious, talkative).

      • Include examples of specific actions, phrases, or tone.

    3. Analyze the Impact on the NP-Client Relationship

      • Consider how the behavior affects trust, rapport, and quality of care.

      • Tip: Think about short-term and long-term effects on communication and patient outcomes.

    4. Apply Communication Strategies

      • Suggest techniques to enhance communication and address the behavior.

      • Examples: active listening, setting boundaries, reflective responses, motivational interviewing, or de-escalation strategies.

      • Tip: Tie your strategies to evidence-based practices and course knowledge.

    5. Create Sample Documentation

      • Document the encounter professionally using subjective and objective information, interventions, and follow-up plans.

      • Tip: Use professional terminology and follow documentation standards as if this were a real EHR entry.

Key Concepts to Focus On:

  • Understanding challenging client behaviors and their impact on care

  • Evidence-based communication techniques for NP practice

  • Professional scenario creation for realistic encounters

  • Proper documentation of interactions in an EHR-style format

  • Reflection on maintaining professionalism and patient-centered care

Sample Solution

Create a scenario depicting an interaction between an NP and a client. Describe the setting and type of encounter.

GL, a 58-year-old male client with newly diagnosed hypertension, a 25-pack-year smoking history, and a BMI of 41, is returning to the primary care clinic for a follow-up after starting hypertensive medications. Upon triage by the nurse, the patient’s blood pressure reads 150/92. When the NP enters the room, the patient appears calm and is pleasant when…….Click below to access the full sample solution (PDF)
