One example of a measurement tool is the Healthcare Effectiveness Data and Information Set (HEDIS) comprehensive care measures. Review the components of HEDIS comprehensive diabetes care; then consider the following scenario.
By Day 3 – Initial Post
As a staff nurse, explain how you would determine how many patients in your practice have diabetes.
Explain how you would determine how many of those patients meet all components of HEDIS comprehensive diabetes care measures.
By Day 7 – Peer Responses
Respond to at least two colleagues.
Critique their data collection mechanisms.
Explain whether their data mechanism is practical.
Discuss how long it might take to gather the data.
Suggest alternatives and/or electronic data collection possibilities.

1: Identifying Diabetic Patients
Determine how diabetes is documented in your practice:
ICD-10 codes in the EHR
Problem lists
Medication lists (e.g., insulin, oral hypoglycemics)
Use EHR reporting tools to generate a patient registry.
Collaborate with IT or quality improvement departments if necessary.
2: Determining HEDIS Compliance
Review HEDIS comprehensive diabetes care components (e.g., HbA1c testing and control, retinal exam, nephropathy screening, blood pressure control).
Use EHR data reports to:
Track laboratory results (HbA1c values)
Verify documented retinal exams
Check nephropathy screening results
Review blood pressure documentation
Compare findings to HEDIS benchmarks.
Identify baseline compliance rates.
